Required Documents for All Campers attending Adventure, Excursion, and New England Sports Camps:

AYCC Liability Waiver

AYCC Medical History Form Camp

**Within the Medical History Form, we also require copies of the camper’s:

• Immunization Records (dated June 1, 2023 or later)
• Sports/Camp Physical Record (dated June 1, 2023 or later)
